How I get your personal information and why I have it
The personal information I process is provided directly by you for one of the following reasons:

Delivering therapy sessions

For invoicing

Notifying GP of therapy sessions

In case of perceived risk to vital health interests
Information will be collected through a personal details form and through initial and ongoing therapy sessions.
I may share this information with your GP or emergency services. In some instances, personal details and therapy notes may be subpoenaed by a Court of Law, and I am obligated to provide this information.
An insurance requirement is that I keep your personal data for 5 years after our sessions end. After this time, your personal data will be deleted.
Under the Data Protection (Jersey) Law, the lawful bases we rely on for processing this information are:
(a) Your consent. You are able to remove your consent at any time. You can do this by contacting sarahhigginstherapy@gmail.com. Please note that, as specified above, your personal data must be kept for 5 years after sessions end.
(b) We have a contractual obligation. Personal data is used in the delivery of therapy sessions.
(c) We have a legal obligation. A court of law may request your personal data from me and I am required to provide this.
(d) We have a vital interest. Should it be considered necessary, the police may be informed of your current mental health state where it is considered there may be a risk to your vital interest.
How I store your personal information

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
